MY HEALTH MYSELF

 

Offered by THINK Hauora.

My Health Myself is a six-week course for people living with one or more long-term conditions (LTC) and their whānau/carers. You can meet new people, learn new skills, share your experiences, and enjoy a cuppa and some kai in a friendly, welcoming group.

What will I get out of the programme?

The My Health Myself Programme uses the 'Stanford University Living a Healthy Life with Chronic Conditions' programme that is an evidence based, internationally recognised self-management course. The programme is designed to help you take care of yourself so that you can live well with your long term condition(s).

 

The course covers the following topics:

 

  • Problem solving

  • Healthy eating

  • Decision making

  • Planning

  • Communication

  • Managing symptoms

  • Managing medications

  • Healthy eating

  • Physical activity

  • Emotions

  • Healthy sleep

  • Breathing and relaxation and so much more

Who is this course for?

If you're over 18 and have a health problem that isn't going to go away, or you live with or care for someone with a long-term condition, then these courses are for you.

 

Examples of long-term conditions include, but are not limited to:

 

  • Arthritis

  • Chronic pain

  • Diabetes

  • Heart conditions

  • Lung conditions

  • Depression

  • Mental health conditions

  • Cancer and its treatments

How can I join the programme?

There is no need for a referral. Simply contact us by phone or email, or talk to your general practice team.

When and where?

Programmes are run throughout the year in the Palmerston North and Horowhenua regions. Occasional programmes may be offered in Ōtaki, Feilding and the Tararua regions. A Pasifika course is also offered 1-2 times a year.

 

Courses are made up of one 2.5 hour session a week, for six weeks.
