Diabetes Information Kits
In collaboration with Diabetes Manawatu the Trust has developed a Diabetes Information Kit that contains all relevant information about diabetes and the support services. This is supplied free to all GP practices and is given to all persons newly diagnosed with diabetes.
The purpose of this kit is to provide sufficient information for the person with diabetes and their family/whanau and to be a resource for future reference. It can be read as required and is not expected to be digested immediately.
